Crossroads

Southern California Crossroads’ mission is to provide safety and healing for individuals and communities who have been impacted by trauma caused by violence. Crossroads is a non-profit 501c(3) organization with over 16 years of proven capacity serving youth and adults in the areas of violence prevention, intervention, hospital-based and school-based services across Los Angeles County and Southern California.

Mentor Program

At Lennox School District, Crossroads provides their student mentoring program. Through this program, student mentors work to prevent violence by intervening in existing issues of student conflict through one on one counseling, mentoring and other youth development approaches. Student mentors work with school administration, educators and parents to provide on campus workshops, services and resource linkage. Student mentors are assigned to campus sites and attend full school days to provide services, safe passages and programming to promote student success.

  • This form is used by administrators and school counselors to refer students to the student mentoring program.

  • This form consents students to receive services. It allows mentors to have one on ones and better support the student.

  • After the consent form has been collected, this assessment tool is completed through an interview-like process between the student and Crossroads staff every 6 months after the initial intake. This consists of about 50-70 questions on: school, self, family, peers, life events, risk behaviors, and social media.
